If You Are Selected to a Team

It is very important that you come to registration (date and time to be determined based on try-outs) at Peppinos Pizzeria & Sports Grille, (1515 Eastport Dr. SE, Kentwood, MI).

If you absolutely cannot attend registration on either night, please contact:

The following items need to be brought to registration if you are selected to a team.

• Copy of birth certificate (If you have never registered with GVSA)
• GVSA Player registration form (Download a copy)
  • If you played for Kentwood Soccer Club in the2010/11 season, we will have a preprinted registration form available for you at registration.
  • Both parent and player signature required on the preprinted form.
• MSYSA Medical Release form - Notarized (Download a copy)
• Passport size photo
• Cash or Check (Payable to Kentwood Soccer Club)
• U15-U18 $275 (half year, one tournament)
